The services of the SmartMe&You-TELEMAIA platform

Nowadays, modifiable risk factors for aging-related dementia can also be measured using widely available and affordable mobile devices and mitigated through personalized interventions for primary prevention in healthy older adults and tertiary prevention in patients with cognitive deficits due to Alzheimer’s disease (AD) and other neurodegenerative diseases.

These concepts inspired the development of the Italian SmartMe&You-TELEMAIA platform, designed, developed in its current version, and clinically validated by the present UNIROMA1 operational unit within the Horizon eBRAIN-Health project (2023–June 2026; https://ebrain-health.eu/), in a European framework and in collaboration with EBRAINS, as well as by UNIROMA1, Sogetel Srl, and Sentech Srl within the TELEMAIA project (2023–2025), funded by the Lazio Region through the “Lazio Innova” call. Validation of the SmartMe&You platform involved approximately 150 older adults at risk or with cognitive deficits due to Alzheimer’s disease, Parkinson’s disease, or Lewy body disease.

The SmartMe&You platform collects, stores, visualizes, and analyzes clinical and instrumental data acquired during one-week home telemonitoring sessions using two commercial devices. A Galaxy Watch smartwatch monitors dementia risk factors, including physical activity, sleep-wake cycle, nocturnal sleep quality, and heart rate variability, which serve as indicators of cardiovascular and autonomic nervous system function. An application (the “SmartMe&You-TELEMAIA app”) can be freely installed on tablets to allow users to play seven serious video games (developed by UNIROMA1) for daily monitoring and training of vigilance, visuospatial attention, and executive functions, each requiring approximately 20 minutes to complete.

The platform also stores hospital clinical records, resting-state EEG data, and structural and functional MRI data, with planned integration into the EBRAINS The Virtual Brain platform. The machine learning models currently implemented on the platform, trained on telemonitoring data, predict MMSE scores with an error of ±1 point, thus proving suitable for clinical follow-up.

The aforementioned clinical validation demonstrated high acceptance among healthy controls and patients and showed significant associations among smartwatch-derived measures, quantitative EEG markers, and neuropsychological scores.

In April 2026, the SmartMe&You-TELEMAIA platform was migrated from the GARR Consortium computing infrastructure to the Cineca Interuniversity Consortium infrastructure. The purpose was to strengthen and better integrate the SmartMe&You-TELEMAIA platform within the European brain research ecosystem (EBRAINS), and to initiate the evaluation process for releasing the platform services within a broader usage framework for citizens, for purposes of telemonitoring and prevention of dementia risk factors, as well as data access by research groups. The platform is now operational on computational resources provided by T-NA, the Italian Research Infrastructure for Neuroscience (IRIN), coordinated by CNR-IBF in Palermo, with CINECA as the implementing body.

The SmartMe&You-TELEMAIA app allows users to test vigilance and attention at their homes with a battery of tests running on standard tablets .

SmartMe&You-TELEMAIA performance data can be immediately visualized on the tablet and is sent to a central repository at GARR Cloud (www.garr.it), the most important academic cloud environment for scientific research in Italy.

SmartMe&You-TELEMAIA App has been

– implemented in the GARR platform by Antonio Calanducci,

– under clinical testing by Roberta Lizio at the Department of Physiology and Pharmacology “Erspamer”, Sapienza University of Rome (Italy).

We are developing procedures for Artificial Intelligence to exploit the clinical applications of markers produced by SmartMe&You, namely the prediction of deficits in brain systems regulating vigilance and attention in Alzheimer’s, Parkinson’s, and related dementia disorders.
